Referral Bonus Request

This form shall be used when an employee has referred someone to Ardent Preschool and Daycare. Detailed information concerning bonuses are in the staff manual and general requirements are listed within this form.

Family Bonus:
The referred family must be in good standing for 30 days and must be a new family. A referral will not be paid if a former family or employee's child is enrolled. Bonuses will go through the payroll system, and will not be issued as a separate check.
Employee Bonus:
The referred employee must be with Ardent for 60 days and have successful 30 & 60 day evaluations. The current employee's name must be listed as "referred by" on the new employee's application, and this form must be submitted within 3 months of the of the new employee's hire date. Bonuses will go through the payroll system, and will not be issued as a separate check. Referrals are for new hires only and will not be paid on former employees who are re-hired.
This form along with all evaluations must be submitted for the employee to be paid.
