Referral Bonus Request

The referred family must be in good standing for 30 days and must be a new family. A referral will not be paid if a former family or employee's child is enrolled. Bonuses will go through the payroll system, and will not be issued as a separate check.

The referred employee must be with Ardent for 60 days and have successful 30 & 60 day evaluations. The current employee's name must be listed as "referred by" on the new employee's application, and this form must be submitted within 3 months of the of the new employee's hire date. Bonuses will go through the payroll system, and will not be issued as a separate check. Referrals are for new hires only and will not be paid on former employees who are re-hired.

This form along with all evaluations must be submitted for the employee to be paid.
